# My Health Membership

> My Health Membership is a monthly healthcare membership for individuals, families and employers in the United States. Members get 24/7 virtual urgent care, virtual primary care, mental health therapy and covered prescriptions with no copays, starting at $19.97 a month. It is not insurance.

Key facts:
- Four plans: Revive Rx (from $19.97/month), Revive Value (from $39.97/month), Revive Care (from $46.97/month), Revive Plus (from $66.97/month). Each plan includes everything in the plan below it.
- Household sizes: Individual, Two Party (you and a spouse or partner) and Family (up to 7 people).
- Free trial: 30 days free on Individual Revive Rx and Revive Value plans. Card required at signup. Your first bill is $0. After 30 days you’re billed monthly for your plan unless you cancel before then. Cancel within your first 30 days and you pay nothing.
- No health questions, no waiting period, no copays or deductibles. Cancel anytime online.
- Care is provided by independent licensed providers by video or phone, in all 50 states. No controlled substances.
- Employer group rates are available; brokers and agents can partner with us.
