Book a visit with a doctor

Request a phone or video visit for urgent care, primary care, therapy and more, depending on your plan.

Steps

Open the member app, or log in to the member portal at https://member.myrevive.health.
Choose the type of care you need, such as urgent care, primary care or therapy.
Choose a video or phone visit.
Pick a time, or request the next available provider for urgent care.
Join the visit from the app or answer the call at your scheduled time.

Urgent care is available 24/7, usually in under 20 minutes.
Primary care, therapy and physical therapy are usually available in 1 to 3 days. Specialists on Revive Plus take about 3 to 6 days.
Doctor visits are included on Revive Value, Care and Plus. Revive Rx covers prescriptions only.
For a medical emergency, call 911.
