Membership terms
Last updated November 2, 2026
These terms are an agreement between you and My Health Membership (“we”, “us”). By enrolling, you accept them on behalf of yourself and every household member you add.
1. What the membership is
My Health Membership is a monthly healthcare membership. It is not insurance and does not replace major medical coverage. Care is delivered by independent licensed providers. Benefits depend on the plan you choose and are described on our plans page. See also our disclosures.
2. Eligibility
The primary member must be 18 or older and live in the United States. Household members are covered as described for each plan: a spouse or domestic partner, children ages 2 to 17 and adult children ages 18 to 26. You agree that the information you give us for each person is accurate and that you have their permission, or are their parent or guardian, to enroll them.
3. Free trial and billing
Individual Revive Rx and Revive Value plans start with a 30-day free trial. A card is required at signup and the first bill is $0. Unless cancelled within the first 30 days, the membership is billed monthly to the card on file starting after the trial, until cancelled. Revive Care, Revive Plus, and all Two Party and Family plans do not include a free trial and are billed monthly starting on the enrollment date, until cancelled.
Failed payments. If a payment fails, we’ll notify you by email (and text, if you opted in) and retry the card over the following days. If we still can’t collect payment, your membership is paused and then ended. You can update your card any time on the My account page.
Price changes. We’ll give you at least 30 days’ notice by email before your monthly price changes. The new price applies from your next billing date after the notice period. If you don’t want to continue at the new price, you can cancel before then.
4. Cancellation and refunds
You can cancel at any time using our cancellation form, by emailing info@myhealthmembership.com or by calling 1-833-589-9957.
- During a free trial: cancel within your first 30 days and you pay nothing. Access ends when the cancellation is processed.
- After the trial, or on a plan without one: we stop your next payment, and your membership stays active through the end of the month you’ve already paid for.
Monthly fees already paid aren’t refunded for partial months, except where state law requires it or where we billed you in error. If you think you were charged by mistake, contact us within 60 days and we’ll review it.
5. Plan changes
To upgrade, downgrade or change your household size, contact us. Plan and tier changes take effect on your next billing date, and the new price applies from then. Household members can be added or removed the same way; a member who ages out of eligibility (for example an adult child turning 27) is removed at the next billing date, and we’ll let you know beforehand.
6. Using the membership
Membership benefits are for the people listed on your account only and can’t be shared or resold. Visits and prescriptions are subject to the clinical judgment of the treating provider and to the limits described for your plan. We may end a membership for fraud, misuse or abusive behavior toward providers or staff.
7. Changes to benefits
Benefits, covered medications and partner networks can change. If we make a change that materially reduces your plan’s benefits, we’ll tell you by email at least 30 days in advance, and you can cancel before it takes effect.
8. State-specific terms
Where a state requires specific notices for membership or discount health programs, those notices are included in that state’s welcome materials and are part of these terms. Nothing in these terms limits any right you have under your state’s law. If any part of these terms is found unenforceable in your state, the rest still applies.
9. Limits of liability
We arrange access to independent providers but don’t provide medical care, and we aren’t responsible for the medical advice or treatment those providers give. To the extent the law allows, our total liability to you for any claim related to the membership is limited to the membership fees you paid in the three months before the claim.
10. Changes to these terms
If we change these terms, we’ll update the date at the top of this page and tell members by email before material changes take effect. Continuing your membership after that date means you accept the updated terms.
