Not insurance
My Health Membership is not insurance. It is a monthly healthcare membership that complements, and does not replace, major medical coverage. It does not meet the minimum creditable coverage requirements of the Affordable Care Act. Care is delivered by independent licensed providers. Benefits vary by plan. Providers do not prescribe controlled substances. Not for emergencies: if you have a medical emergency, call 911.
The membership does not pay for hospital stays, surgery, emergency room visits, ambulance services or in-person specialist care except where a plan specifically lists an in-person benefit. If you need comprehensive coverage, keep or buy a major medical insurance plan. Membership fees are not insurance premiums and are not a substitute for health insurance.
Who provides care
My Health Membership is not a healthcare provider and does not practice medicine. Virtual visits, therapy, prescriptions and other clinical services are provided by independent, licensed clinicians through a national care platform. Those clinicians make their own medical decisions, including whether a virtual visit is appropriate and whether to prescribe a medication. A provider may refer you to in-person care when that is the right choice for your health.
Availability of specific services, specialists and appointment times can vary by state and by plan.
Prescriptions
Covered medications are limited to those on the membership medication list, which can change. Prescriptions are written only when a licensed provider decides one is appropriate. Home delivery and pharmacy pickup are subject to the limits described for each plan.
Controlled substances
Providers do not prescribe controlled substances (for example opioids, stimulants, benzodiazepines or sleep medications classed as controlled) through the membership.
Pharmacy discount card
The pharmacy discount card included with every membership is not insurance. It provides discounts on prescriptions that are not on the covered medication list, at participating pharmacies only. You pay the discounted price directly to the pharmacy. Savings vary by medication and pharmacy and are not guaranteed, and the card cannot be combined with insurance for the same purchase.
Emergencies
The membership is not for emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency room. If you are in crisis or thinking about harming yourself, call or text 988 to reach the Suicide and Crisis Lifeline.
Your privacy
We don’t collect health information on our website, and we don’t receive your medical records. Clinical information you share during care is held by the providers under their own HIPAA notice of privacy practices. See our privacy policy.
Pricing and free trial
Prices shown are monthly membership fees for the plan and household size listed. The 30-day free trial applies only to Individual Revive Rx and Revive Value plans. Full billing, cancellation and refund terms are in the membership terms.
State notices
The membership is offered in all 50 states and the District of Columbia. Some states regulate discount and membership health programs and require specific notices. Residents of those states can find the notice that applies to them in the membership terms and in their welcome materials, or request a copy by contacting us.
Questions or complaints can be sent to us first at info@myhealthmembership.com or 1-833-589-9957. You may also contact your state’s department of insurance or attorney general’s consumer protection office.
