Choose your plan and who it covers, then check out. It takes about five minutes.

Log in to your member portal
Within 48 hours you’ll get a welcome email. Use it to log in to your portal and download the app.
Within 48 hours - Step 3
Schedule an appointment with a real doctor
Feeling sick or need a refill? Book a phone or video visit in the portal. Once you’re live, you can talk to a doctor the same day.
Same-day visits - Step 4
Talk to your doctor
Explain what’s going on from home, work or anywhere. Your doctor recommends next steps and writes prescriptions if you need them.
Phone or video - Step 5
Get your prescriptions
Have them sent to your local pharmacy for pickup today, or delivered to your home. Covered medicines cost nothing extra.

My Health Membership is not insurance. It’s a monthly membership for virtual doctor visits and prescriptions that works alongside your insurance. Please read this before you join.
What it is
- A monthly membership for virtual doctor visits and prescriptions
- Care by phone or video with licensed doctors and therapists from our network
- A way to work alongside your current insurance, or to use on its own between plans
- Medicines on our list at no extra cost, with free home delivery
- No copays and no deductible for the services your plan includes
- Month to month. Cancel anytime
What it isn’t
- Not health insurance, and not a replacement for it
- Mostly virtual care. Visits are by phone or video. Only Revive Plus includes in-person care: 2 free visits a year at CVS MinuteClinic, plus a free flu shot
- You can’t use your current doctor. Visits are with providers from our network. (Revive Rx can still fill prescriptions from your own doctor.)
- Doesn’t pay for ER visits, hospital stays, surgery, imaging or outside specialists
- Not for emergencies. Call 911
- Only medicines on our list. No controlled substances (like narcotics or stimulants) or high-cost specialty drugs
- A visit doesn’t guarantee a prescription. Your doctor decides what’s medically right for you

- Want quick care for everyday things like colds, infections, rashes or refills
- Take maintenance medicines that are on our list
- Are comfortable talking to a doctor by phone or video
