Privacy policy
Last updated November 2, 2026
This policy explains what information My Health Membership (“we”, “us”) collects through myhealthmembership.com and our enrollment, contact and cancellation forms, how we use it, and the choices you have. It applies to this website and to account emails and texts we send you.
The short version
- We collect only what’s needed to set up and bill your membership: names, dates of birth, gender, contact details, mailing address and preferred language for each person covered.
- We don’t collect health information. We never ask about medical conditions, symptoms, medications, diagnoses or medical history, and we don’t receive or store your medical records.
- We don’t collect Social Security numbers, insurance member IDs or driver’s license numbers.
- Card details go straight to our payment processor. We never see or store your full card number.
- We don’t sell your personal information or use it for targeted advertising.
Health information and HIPAA
My Health Membership is a membership program, not a healthcare provider or health plan. We don’t provide medical care, and our website and systems are not used to collect, store or process protected health information (PHI) as defined by the Health Insurance Portability and Accountability Act (HIPAA).
When you use your membership, you deal directly with independent licensed providers through the member portal and app. Anything you share during a visit, such as symptoms, prescriptions or visit notes, is held by those providers and their care platform under their own HIPAA notice of privacy practices, which you’ll see when you set up your member portal. We don’t have access to it.
Please don’t send medical details through our contact forms, email or texts. We only need enough to point you in the right direction.
What we collect
Information you give us
- Enrollment: for the primary member and each household member, first and last name, date of birth, gender, email, mobile phone, mailing address and preferred language, plus the plan and household size you choose.
- Payment: you enter your card on our payment processor’s secure checkout. We receive a confirmation, the last four digits and the card brand so we can show you which card is on file.
- Contact, employer, broker and cancellation forms: name, email, phone, company name and size, and the message you send.
- Consents: a record that you accepted the membership terms and, if you choose, agreed to receive texts.
Information collected automatically
- Referral codes: if you arrive from a link with a campaign or broker code (for example
utm_sourceor?broker=), we keep that code in your browser for the current session only and attach it to your enrollment so the right partner gets credit. - Basic technical data: like any website, our hosting provider receives your IP address, browser type and the pages you request in standard server logs, used for security and to keep the site running.
- Fonts: our pages load fonts from Google Fonts, which receives your IP address when the font files are requested.
We don’t use advertising cookies, tracking pixels or cross-site tracking.
How we use it
- Set up your membership and the household members you add, and activate your access to the care network.
- Bill your membership, apply any free trial, and send receipts.
- Send account messages: welcome and setup emails, billing notices, and texts if you opted in.
- Answer your questions, process cancellations and plan changes, and provide support.
- Credit the employer, broker or campaign that referred you.
- Keep the site secure, prevent fraud, and meet legal and accounting obligations.
Who we share it with
We share information only with the companies that help us run the membership, and only what each one needs:
- Care platform and provider network: the eligibility details needed to activate each covered person (name, date of birth, gender, contact details, address, preferred language and plan). This is how your member portal and app account get created.
- Payment processor: to take payments, store your card securely and run the billing portal where you update your card.
- Customer relationship and messaging provider: to keep your account record and send our emails and texts.
- Your employer or broker: if you join through an employer group or a broker, they may see that you enrolled, your plan and your billing status. They don’t see anything about care you receive.
- Legal and safety: when required by law, to respond to valid legal process, or to protect the rights and safety of our members and our business.
- Business transfers: if My Health Membership is merged or sold, your information may transfer to the new owner under this policy.
We don’t sell personal information, and we don’t share it for cross-context behavioral advertising.
Text messages
If you check the text-message box at enrollment, we’ll send setup reminders and account updates to the mobile number you gave us. Message frequency varies, typically a few messages during setup and occasional billing or account notices after that. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. Texting is optional and isn’t required to join.
We don’t share your mobile number or text-message consent with third parties or affiliates for their marketing.
How long we keep it
We keep your account information while your membership is active and for as long as needed afterward to meet tax, accounting and legal requirements, usually up to seven years for billing records. Contact-form messages from people who don’t join are deleted within two years. Then we delete or de-identify the information.
How we protect it
Our website is served only over encrypted HTTPS connections. Card data is handled by a PCI-compliant payment processor. Dates of birth, addresses and household details are never put in web addresses or saved in your browser. Access to member records is limited to staff and providers who need it to support you. No system is perfectly secure, but we work to protect your information and will notify you if a breach affects you as required by law.
Your choices and rights
Wherever you live, you can ask us to:
- Tell you what personal information we have about you and give you a copy.
- Correct information that’s wrong.
- Delete your information, except what we must keep for legal or accounting reasons.
- Stop sending you texts or marketing emails. Account and billing notices continue while your membership is active.
Residents of California, Colorado, Connecticut, Virginia, Texas, Oregon and other states with consumer privacy laws have these rights under state law, including the right to know, correct and delete and to opt out of sale or targeted advertising (which we don’t do). You can use an authorized agent; we may ask them to show your written permission. We’ll verify your request by matching details already on your account, respond within 45 days, and won’t treat you differently for using your rights. If we deny a request, you can appeal by replying to our decision.
To make a request, email info@myhealthmembership.com or call 1-833-589-9957.
Children
This site is meant for adults. A parent or guardian enrolling a household may add children ages 2 to 17 to their membership, and we collect only the enrollment details listed above for them. We don’t knowingly collect information directly from anyone under 18.
Links to other sites
The member portal, app, payment checkout and billing portal are run by our partners and have their own privacy policies. This policy doesn’t cover them.
Changes to this policy
If we change this policy, we’ll update the date at the top of this page. If a change materially affects how we use information we already have, we’ll tell members by email before it takes effect.
