Cancel your membership
Cancel any time online, by email or by phone.
Steps
- Go to https://myhealthmembership.com/cancel.
- Enter your name and the email and phone on your membership.
- Choose your plan and the main reason you're leaving.
- Select Cancel my membership.
- Watch for an email confirming your cancellation within one business day.
Good to know
- On an Individual Revive Rx or Revive Value plan, cancel within your first 30 days and you pay nothing.
- Otherwise, your membership stays active through the end of the period you've already paid for.
- Fees already paid aren't refunded for partial months, except where state law requires it or we billed you in error.
